Not sure if you're up for it, but make Google mistranslate to different languages and back until the text is terrible in various text boxes. Or at least the ones you're likely to see.
Change the palates of the OW randomly to other palates from another OW.
Possibly randomly pick palettes for the stages in Yoshi's Island, and Valley of Bowser.
My final change is adding a single yellow switch block tile under Mario and an invisible block above Mario in Donut Plains 1 so the player is required to get the yellow switch.

Make Yoshi's House only be exitable by death;
Make Donut Secret House's door leading to the big boo boss send you to a test level (and put the door to the boss in a random place if you want);
Make Tubular unbeatable unless you have all switch palaces (put the switch blocks as the ground after the goal point).
Put Bowser's theme (the main part that starts very loud) in Donut Plains 1.
make all the switch palaces have munchers everywhere
make it impossible to get capes until vanilla dome
delete all powerups for 105

SMW but Mario has his infamous SMB1 Physics
Put a hole right in the beginning of YI1 and DP4's bonus rooms.
Move message blocks of YI4 to the main level into whatever places you want.
Replace all normal goals with the "goal sphere orb" and hide the orb in every level in a neat place that is different than where it currently is.
So normally you just have to get to the end of a level and get the normal exit, but now you have to find the orb and the orb can be anywhere in the level. Do this for every playable level that has a normal exit.
Use the goal sphere in the levels mentioned above even if the orb doesn't use the proper graphics.
1. Remove all platform sprites from level 005
2. Globally change the back area 3 color 0 | 255 | 255
3. Use a different port of yours for every single room in Bowser's castle (aka Shit House now)
Change the first area of Gnarly (level 12A) from a vertical level into a horizontal level, while leaving all objects and sprites where they end up. (Also, perhaps assign an exit to level 1C4 from screen 05 if you do this, so that the pipe there doesn't lead to a bonus game trap.)
Make 90% of sprites be Footballs.
Make all switches create 4 tiles high walls in very annoying places
put invisible 1f0 tiles all over the place
use old addmusic to make the music break
