Nowadays, ESRB would probably rate them at least E10+, look at Triple Deluxe and tell me it deserved a 10+ rating. It is exactly as "violent" as its predecessors and these got an E. Since we're crushing enemies with out bare boots in our hacks, they would deserve nothing less than E10+ in modern ESRB.
AO wouldn't be appropriate for any SMW hack. It is mainly reserved for [naughty word] games

Considering the common ratings of Square-Enix RPGs, it'd probably be an E10+ or T (a 12/16 in PEGI's case)
There is a Vita remake of that game coming out like this year or so, so the Vita version's CERO rating could be used as an indicatorHack・Ports・ASM・"Uploader"

I would say, most hacks would be something like E6+ or E10+ but probably quite a few hacks might come to the T13+ area (e.g. Mario Gives Up). I would also think that the USK (Germany's own rating system) and PEGI (rest of Europe) would rate most of them with 0 or 6 years and those, which would be likely rated as T and probably some kind of E10+ in the U.S., 12 years.
Of course, some hacks are exception and would be probably marked as M17+/ 16 (e.g. horror hacks like 'the comes in mind) and would come with much luck as T13+/ 12 away.
